Transition Case Management provides short term help to families with a child, who is making the important transition from the Gateways Early Childhood Intervention Program to school age services.

What is Transition Case Management?

Transition Case Management provides short term help to families with a child, who is making the important transition from the Gateways Early Childhood Intervention Program to school age services.

What does a Transition Case Manager do?

Transition Case Managers support families during this time of change by providing helpful background information about the range of services and supports available to the family.

Transition Case Managers also work in partnership with each family to plan and coordinate the range of services and supports they wish to use.

The partnership between the Transition Case Manager and the family is based on respect for the family’s rights, wishes and strengths.

Who is eligible for this type of support?

Families with a child aged five to eight years with Autism Spectrum Disorder may be eligible for Transition Case Management Support.

Priority is given to clients and their families where:

  • They have a child aged between five to eight years, who has recently made the transition from Gateways Early Intervention Services to school.
  • A coordinated support strategy is needed to assist the family during this transition period.

What kinds of issues can Transition Case Management help to address?

Support, for up to six months, can address a wide range of issues including:

  • Parenting strategies for children with an Autism Spectrum Disorder.
  • Behaviour management strategies.
  • Information about other service options, including respite and case management.
  • Parent support.
  • Educational options and program guidelines.
  • Provision of advocacy support.
  • Support to siblings.
  • Support to other professionals working with children and families.
  • Information provision on Autism Spectrum Disorder.
