Short Term Case Management
Case managers help families with children aged between nine and 18-years with an Autism Spectrum Disorder access information and resources.

What is Short Term Case Management?

Short Term Case Management provides support to families with a child or young person with an Autism Spectrum Disorder by assessing the family’s needs, then coordinating and linking services, referrals, programs and activities to meet these needs.

At Gateways case managers work in partnership with families, on a short term basis, to assess their situation and identify the ongoing services they need to live a positive and happy life in their community.

When formulating a plan, the case manager will draw on many sources including information from the family, other service providers and literature on the needs and appropriate strategies that are effective in working with children and adolescents with an Autism Spectrum Disorder.

Families can be assured that confidentiality is maintained and information is not forwarded to other services or agencies without the consent of the family.

Who is eligible for Short Term Case Management?

Families may be eligible for Short Term Case Management services if they have a child aged between nine and 18-years with a diagnosed Autism Spectrum Disorder, including Asperger Syndrome.

Priority is given to clients and their families where:

  • The client is not eligible for other case management services.
  • The child and family have complex and ongoing support needs; and,
  • The child and/or family are considered to be at risk.

These families must also have complex needs defined as:

  • The family needs a range of services on an ongoing basis.
  • The family is not coping; and,
  • There are few existing services that the family can utilise.

What sort of advice is offered?

Case managers work with families on a short term basis and offer advice in relation to individuals with an Autism Spectrum Disorder in the following areas:

  • Parenting strategies for children with an Autism Spectrum Disorder.
  • Behaviour management.
  • Service options, including respite and case management.
  • Service access.
  • Parent support.
  • Support to siblings.
  • Support to other professionals; and,
  • Information about Autism Spectrum Disorders.
