At Gateways Support Services, we understand the importance of making the right referral, one that leads to meaningful, life-enhancing outcomes for the people and families you support. 

Gateways is committed to making the referral process as simple, responsive, and person-centred as possible. Whether you’re referring a child, adult or family member, our team is available to guide you through the process and ensure each person is connected to the most appropriate supports. 

Referrals can be made by: 

  • Support Coordinators 
  • Allied Health Professionals 
  • Early Childhood Educators 
  • School Staff 
  • Case Managers 
  • Parents or Carers 
  • NDIS Participants 

The most common referrals are for the following key services: 

Support Coordination 
Support Coordination helps people understand their NDIS plan and connect with providers who can deliver the services they need. Our coordinators assist with navigating options, building capacity, and ensuring each person gets the most out of their plan. 

Individual Support 
Tailored, one-on-one support based on a person’s unique needs and goals. Whether it’s help with daily tasks, building skills, or community participation, our team works closely to deliver consistent, person-centred support. 

Recreation Programs 
These programs give individuals the opportunity to explore new interests, make friends, and enjoy inclusive community activities in a safe and supported environment. 

Positive Behaviour Support 
We work with individuals, families, and support networks to understand behaviour and develop strategies that improve quality of life and reduce restrictive practices. Our experienced team builds positive, strengths-based plans tailored to each person.
