People with an NDIS Plan
Currently we are not taking on new participants for Individual Support Services but we do offer a range of group based recreation programs for adults with a disability, which also provides families and carers with a break from their caring role. People who are Frail Aged or have Dementia
Individual support services are available in the Geelong region for adults over 65 years with a referral from MY Aged Care.
Gateways has experienced staff to support the participant in their home or community and assists the primary carer by providing some respite so they can continue to care for someone who is frail aged. In consultation with each family, Gateways develops a personalised individual support plan that ensures that the services the participant receives addresses their needs.
Who is eligible for this type of individual support?
Families caring for a person aged 65-years or more, who is frail aged, are eligible for referral to this program.
How much individual support may we be entitled to and how much will it cost?
An allocation of hours will be provided to eligible families each year. Generally, up to five hours a week is allocated. Families have the flexibility to choose when these hours are used during the week. A standard fee of $3.90 per hour is charged for this type of service.
This service is supported by the Australian Government Department of Health.
Gateways also runs a range of recreation and social programs in Geelong and Western Melbourne for people who are frail aged or have dementia.
